Level Select:

At the title  screen, press Left, Up, Right, Down, Down, Right, Right, Up, and R.

999 Lives:

At the title screen, press Up, Right nine  times, Down six times, Left seven  times, Z, X, Y, Y, Y, Z.  Start a  game  and you'll have almost unlimited lives.

Stage select:

Press Left, Up, Right, Down(2), Right(2), Up, R at the title screen while the phrase "Press Start Button" is displayed. The words "Betsy's Room" will appear. Press Up and Down to choose a stage.

Final stage:

Enable the "Stage select" code. Instead of pressing Up or Down to select a stage, press Left, Right(2), Up, Right(2), Up, Down, Right(2),Up, R. Then, press Up(2) to display the "Last Boss" option on the stage selection screen.

999 lives:

Press Up, Right(9), Down(6), Left(7), Z, X, Y(3), Z at the title screen while the phrase "Press Start Button" is displayed. The opening theme song will restart to confirm correct code entry.
